INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position at Northwest Hospital in Randallstown, MD involves providing discharge planning coordination and interventions in collaboration with the clinical team and medical providers. The role focuses on promoting patient wellness, improving care outcomes, ensuring efficient utilization of health services, and reducing payment denials for patients with complex health needs.

Requirements

• Bachelor's degree in a relevant field.
• BSN required from CCNE accredited schools.
• Prior experience in a similar role is a plus.
• Excellent communication and interpersonal skills.
