Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face. This position involves supporting individuals in adjusting to institutional living, addressing financial concerns, assisting with family relationships, and providing counseling on end-of-life matters.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist individuals in dealing with adjustment issues related to medical conditions and aging.
- Collaborate with healthcare teams to develop effective discharge plans.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Preferred certifications include LCSW-C and LGSW.
- Strong communication and interpersonal skills.
