Job Description
We are looking for an Inpatient Case Manager (RN) to join our team at LifeBridge Health. As part of the care management team at Sinai Hospital in Baltimore, MD, you will collaborate with clinical teams and medical providers to provide discharge planning coordination and interventions for patients with complex health needs. The ideal candidate will promote patient wellness, improve care outcomes, ensure efficient health service utilization, and minimize payment denials.
Responsibilities

Requirements
- Bachelor's degree or equivalent professional knowledge.
- BSN preferred; ADN required.
- 3-5 years of related experience.
