Job Description
We are looking for a dedicated Hospice Social Worker to join our team at LifeBridge Health for weekend shifts only. As a key member of our hospice care team, you will provide professional and compassionate services to individuals and their families in the home setting.
Responsibilities
- Assess psychosocial, environmental, and financial needs of patients and families.
- Collaborate with patients, families, and caregivers to develop and implement comprehensive care plans.
- Offer counseling and support to help cope with terminal illness and the grieving process.
- Coordinate referrals for community and agency homecare services.
Requirements
- Master's Degree in Social Work and relevant certifications (LCSW, LCPC).
- Minimum of three years of clinical experience, with community health experience preferred.
- Strong interpersonal communication skills and the ability to maintain accurate documentation.
- Professional licensure in Maryland and Pennsylvania.
