Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling. Responsibilities
- Provide psychosocial assessments to patients/residents and families.
- Offer counseling on issues related to aging, powerlessness, and end-of-life care.
- Assist with discharge planning and support patients through medical procedures.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Strong communication and interpersonal skills.
