Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an integral part of the clinical team, you will collaborate with medical providers to ensure effective discharge planning coordination and interventions for patients with complex health needs.
Responsibilities
- Provide discharge planning coordination and intervention.
- Promote patient wellness and improved care outcomes.
- Ensure efficient utilization of health services.
- Minimize denials of payment among a patient population with complex health needs.
Requirements
- Bachelor's degree or equivalent professional knowledge.
- BSN preferred; ADN required.
- Maryland Registered Nurse License.
- Case Management certification required within 3-5 years of hire.
