Job Description
We are looking for a dedicated Inpatient Case Manager - Social Worker with an LMSW certification to join LifeBridge Health at Sinai Hospital in Baltimore, MD. This full-time position with a weekend commitment involves day shifts from 8:00 am to 4:30 pm in the Care Management department.
Responsibilities
- Provide patient and family advocacy in collaboration with the clinical team and medical provider.
- Coordinate discharge planning and offer psychosocial interventions for high-risk inpatients.
- Promote patient and family wellness, improved care outcomes, and access to appropriate resources.
Requirements
- Master's degree in Social Work required.
- 2 years of experience preferred in hospital social work or community social services.
- LMSW required; LCSW/LCSW-C preferred.
- Maryland Social Work License as per level of education.
- Effective verbal and written communication skills.
