INPATIENT CASE MANAGER (RN) - PRN

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. The Inpatient Case Manager will collaborate with the clinical team and medical provider to provide discharge planning coordination and intervention for a patient population with complex health needs. Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical provider.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patients with complex health needs.

Requirements

• Bachelor's degree in Nursing (BSN preferred).
• 3-5 years of relevant experience.
• American Heart Association CPR Certification.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire for the role.
