Job Description
We are looking for a compassionate and skilled Social Worker to join our team at LifeBridge Health. As a Social Worker at Levindale Hospital in Baltimore, MD, you will be responsible for providing clinical social work services to patients and their families, addressing various challenges such as adjustment to medical procedures, aging-related changes, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide support to patients and families.
- Assist with discharge planning and address financial and family-related concerns.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Preference for candidates with Licensed Certified Social Worker-Clinical (LCSW-C) designation.
