Social Worker at LifeBridge Health
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges they may face. This full-time position is based at Levindale Hospital in Baltimore, MD, working the day shift from 8:00am to 4:30pm.
Responsibilities
- Provide counseling and support to patients and families on various issues such as adjustment to institutional living and end-of-life care.
- Assist in discharge planning and address financial and family relationship concerns.
- Collaborate with medical teams to ensure optimal patient care and well-being.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
