Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health in Baltimore, MD. As part of our team, you will be responsible for providing clinical social work services to patients and families, addressing a range of psychosocial issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life counseling.
- Address concerns related to institutional living, financial issues, and family relationships.
- Collaborate with healthcare professionals to support patients' well-being.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW required.
- Experience in clinical social work and counseling.
- Strong communication and interpersonal skills.
