Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common issues such as adjustment to institutional living and family relationships.
- Offer counseling related to death and dying.
- Engage in discharge planning.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification.
