Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues related to medical care, aging, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and facilitate communication between patients and healthcare providers.
- Address financial concerns and help patients navigate institutional living adjustments.
- Offer support and guidance on family relationships and acceptance of medical treatments.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong interpersonal skills and ability to handle sensitive situations with empathy.
