Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a member of our team, you will provide clinical social work services and psychosocial assessments to patients and their families, assisting with a variety of challenges they may face.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address common issues such as adjustment to institutional living, financial concerns, and discharge planning.
- Collaborate with healthcare professionals to ensure comprehensive care for patients.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
