Job Description
We are looking to hire a dedicated and compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. This position involves working with individuals on adjustment to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.
Responsibilities
- Provide clinical social work services to patients, residents, and families.
- Assess and address common social and emotional challenges faced by patients.
- Offer support and counseling related to various issues, including aging and end-of-life care.
- Collaborate with healthcare teams to ensure comprehensive care for patients.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Prior experience in a healthcare setting is a plus.
