Job Description
We are looking for a passionate Social Worker to join LifeBridge Health in Baltimore, MD. As part of our team, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges and offering support. Responsibilities
- Provide clinical social work services, including psychosocial assessments.
- Assist patients and families with adjustment to institutional living, financial concerns, family relationships, and more.
- Offer counseling on discharge planning, death and dying, and other sensitive topics.
- Collaborate with healthcare teams to support holistic patient care.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) certification.
