Job Description
We are looking for a dedicated Social Worker to join the team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to various life transitions.
Responsibilities
- Offer clinical social work services to patients and families.
- Assess and address a variety of psychosocial issues.
- Provide counseling and support for individuals and families facing challenging situations.
- Assist with discharge planning and end-of-life care decisions.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
