Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a Clinical Social Worker, you will provide essential social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face. This role involves assisting with adjustment to living in an institutional setting, financial matters, family dynamics, coping with medical conditions, aging-related issues, discharge planning, and grief counseling.
Responsibilities
- Conduct psychosocial assessments and create treatment plans.
- Assist patients and families in coping with emotional and social challenges.
- Collaborate with healthcare teams to provide comprehensive care.
- Facilitate referrals to community resources and support services.
Requirements
- Bachelor's degree in Social Work (BSW).
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification required.
