Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a vital member of our allied health professionals, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common problems faced by patients/residents and families.
- Offer counseling and support for a range of issues such as financial concerns and discharge planning.
Requirements
- Bachelor's in Social Work (BSW) required. Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
