Job Description
We are looking for a passionate and dedicated Social Worker to join LifeBridge Health, one of the largest healthcare providers in Maryland. As a Social Worker at Levindale Hospital in Baltimore, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients/residents and families in coping with various challenges and transitions.
- Participate in discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification.
- Experience in healthcare settings is a plus.
