Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, and family relationships. You will play a crucial role in discharge planning, counseling for end-of-life care, and supporting individuals through difficult transitions.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Assist patients with adjustment to institutional living and coping with medical diagnoses.
- Engage in discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to address patients' and families' needs.
Requirements
- Bachelor's degree in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
