Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, family dynamics, healthcare procedures, and end-of-life issues.
Responsibilities
- Offer psychosocial support to patients and families facing medical, financial, and emotional difficulties.
- Conduct assessments and provide counseling to facilitate coping and adjustment.
- Collaborate with healthcare teams to develop discharge plans and ensure continuity of care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree and LCSW-C certification preferred.
- LGSW certification.
- Strong communication and interpersonal skills.
