Curana Health is seeking a dedicated Nurse Practitioner or Physician Assistant to provide exceptional patient care services within our Post-Acute Care setting. Responsibilities

• Conduct direct care visits and re-evaluations for residents.
• Evaluate and treat chronic illnesses and changes in patient conditions.
• Collaborate with physicians and other healthcare professionals for coordinated care.
• Perform Annual Wellness Visits and History and Physicals as needed.
• Communicate with families and attend conferences when possible.
• Additional duties as assigned.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations with current board certification.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to work within a team.
