Job Description
Northwest Kidney Centers is looking for a dedicated Social Worker to join our team in a full-time, variable position. This role involves providing social work intake and patient advocacy services, coordinating social services among various stakeholders, supporting patient education, and collaborating with the interdisciplinary team.
Responsibilities
- Conduct comprehensive social service needs assessments for each patient
- Develop individualized care plans in accordance with ESRD Conditions of Coverage
- Educate patients on their rights and grievance process
- Participate in Quality Assessment Performance Improvement activities
- Collaborate with the team to ensure quality patient care
Requirements
- MSW degree
- Current WA Social Worker License
- Previous medical social work experience (preferred)
- Valid Washington Driver's License, reliable transportation, and car insurance
