Primary Care Physician (Bilingual Spanish)

Job Description

Esperanza Health Centers is seeking a dedicated and compassionate Primary Care Physician to join our team. In this role, you will provide primary medical care services to older adult participants enrolled in our Program of All-Inclusive Care for the Elderly (PACE). As a Primary Care Physician, you will focus on prevention, healthy lifestyle promotion, and coordination of care within our interdisciplinary team.

Primary Duties and Responsibilities

• Manage primary care practice in PACE Center, homes, and post-acute facilities.
• Coordinate with the interdisciplinary team to develop individualized care plans.
• Participate in quality improvement activities and utilization management.
• Communicate with partner facilities to ensure continuity of care.

Requirements

• Bilingual in English and Spanish.
• Board certification in primary care, geriatrics specialization preferred.
• State of Illinois physician license.
• Valid driver's license and automobile insurance.
• Compliance with vaccination requirements for service locations.
