INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join LifeBridge Health at Northwest Hospital. As a full-time employee with a weekend commitment, you will play a crucial role in discharge planning coordination and intervention in collaboration with the clinical team and medical providers. Your focus will be on promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.

Requirements

• Basic professional knowledge with a Bachelor's degree; BSN required from CCNE accredited schools.
• Experience in discharge planning coordination is a plus.
• Strong communication and collaboration skills.
