HOSPICE CLINICAL MANAGER - RN

Job Description

Liberty Healthcare Management is currently seeking an experienced Hospice Clinical Manager - RN to join our compassionate team. As the Clinical Manager, you will coordinate case management, supervise staff, provide direct patient care, and ensure efficient utilization of resources. This full-time position is located in Cumberland County.

Responsibilities

• Coordinate case management of patient services.
• Ensure accurate documentation for patient care and staff performance.
• Supervise assigned Care staff.
• Provide direct patient care as needed, including case management and Oasis care planning.

Requirements

• Current RN licensure in North Carolina.
• Minimum of three years clinical nursing experience, hospice experience preferred.
• Deep understanding of hospice/homecare regulations.
• Strong critical thinking and communication skills.
• Proficiency in computer applications.
• CPR certification required.
