Job Description

Liberty Hospice is seeking a dedicated HOSPICE MEDICAL SOCIAL WORKER to join our compassionate team. As a full-time member covering Cumberland County, you will provide direct patient care services, develop individualized care plans, assess psycho-social needs, and offer bereavement support to patients and caregivers.

Responsibilities

• Provide direct patient care services and document patient care data.
• Assess patients for individualized care plans and treatment goals.
• Identify social, emotional, financial, and environmental factors impacting treatment.
• Coordinate with agency personnel and other services for optimum care.
• Assist with homecare visits and offer bereavement support.
• Utilize community resources and educate patients on resource utilization.

Requirements

• Master's degree in Social Work from a CSWE-accredited school.
• Strong interpersonal skills for effective communication in a multidisciplinary environment.
• Willingness to travel and adapt to varied working conditions.
• Maintain twelve hours of continuing education annually.
