Job Summary

We are looking for a dedicated Case Manager to join House of Hope Green Bay, Inc, to oversee case management duties and program delivery for clients in our Emergency Shelter and Housing Stability programs. The successful candidate will play a crucial role in ensuring that clients receive the support needed for their success while maintaining compliance with legal requirements and grant standards.

Hours

This is a full-time salaried position that requires flexibility in working hours, including weekends and holidays when necessary.

Essential Duties

  • Provide case management activities, housing navigation, and case plan oversight.
  • Collaborate with Advocates and Lead Case Manager for effective client service planning.
  • Ensure high-quality opportunities for youth and young families in parenting, education, and stable housing.
  • Screen clients for program eligibility and coordinate with local agencies.
  • Maintain accurate client records and case notes as per policies.
  • Provide crisis intervention and ensure a safe environment in the shelter.
  • Propose program modifications based on client and community needs.
  • Participate in community events and training sessions.

Requirements

Valid Wisconsin driver's license, background check, and degree or work experience in social services required. Excellent communication skills, flexibility, and knowledge of housing programs preferred.

Physical and Mental Demands

The role requires physical and mental fitness, including the ability to lift 50 lbs, operate office equipment, and maintain good health per regulatory standards.
