Care Manager

The Agency on Aging of South Central CT is seeking a Care Manager to join our team. In this role, you will be responsible for assessing the health, functional, and cognitive status of clients, determining their need for home care services, and assisting them in accessing available resources and programs.

Responsibilities

  • Complete annual assessments and paperwork for continued services.
  • Educate clients on available resources and services.
  • Assist clients in completing applications for supportive services.
  • Monitor and adjust care plans as needed.
  • Ensure timely documentation and reporting.

Requirements

  • Bachelor's degree in health, social work, nursing, or related field.
  • Minimum of one year of experience working with older adults or individuals with disabilities.
  • Excellent interpersonal and organizational skills.
  • Experience with community resources and state-funded programs.
  • Bilingual skills are a plus.
