Registered Nurse - Intake & Hospital Liaison

Job Description

We are looking for a dedicated Registered Nurse to join our Intake and Hospital Liaison team at UVM Health Network Home Health & Hospice. In this critical role, you will serve as the primary contact point between referring providers, program leaders, clinicians, and patients, ensuring a smooth transition to our services. The position involves engaging in a blended role which includes on-site duties at UVM Medical Center and remote responsibilities.

Responsibilities

• Educate patients and referring providers about our services.
• Coordinate patient discharges and transitions to home health and hospice care.
• Review clinical documentation and assess patient eligibility for services.
• Write orders for care and provide information to patients and families.

Requirements

• Associate's degree in nursing and an RN license.
• Minimum of two years of experience as a Registered Nurse.
