Hospice Registered Nurse

Job Description

We are currently seeking a dedicated Hospice Registered Nurse (RN) to join our compassionate team at Liberty Healthcare Management. As a Hospice RN, you will provide state-of-the-art care and pain management services to our patients. Your role will involve applying nursing theories, coordinating patient care, and maintaining documentation that meets reimbursement guidelines. Additionally, you will collaborate with patients, families, physicians, and the Hospice team to ensure comprehensive care and support.

Responsibilities

• Apply nursing theories and concepts in patient care.
• Coordinate and provide physical care in collaboration with the patient's healthcare team.
• Conduct effective communication with patients, families, and agency personnel.
• Participate in interdisciplinary team meetings and quality reviews.

Requirements

• Current RN licensure in the state of practice.
• Minimum two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
• Knowledge of home health and hospice care programs.
• Familiarity with nursing principles, agency policies, and reimbursement guidelines.
• CPR certification and commitment to ongoing education.
• Willingness to travel as per agency requirements, including evening, weekend, and holiday shifts.
