Job Description
We are currently seeking a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. This full-time position will involve providing skilled nursing care and treatment to geriatric patients in the community. As part of our supportive and advanced HomeHealth team, you will develop care plans tailored to each patient's needs, oversee patient discharge planning, and maintain IV proficiency. Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Develop and implement patient-centered care plans.
- Coordinate patient discharge planning.
- Maintain IV proficiency and perform IV functions.
Requirements
- BSN required; ADN candidates must obtain BSN within three years.
- Current DE RN license or compact state license.
- Minimum 2 years of recent HomeHealth RN experience.
- Prior experience in Home Healthcare and Med-Surgical settings preferred.
