Job Description
We are looking for an experienced Licensed Practical Nurse (LPN) to join our dedicated team at ChristianaCare HomeHealth. You will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. This position offers both Full Time, Part Time, and Per Diem opportunities with flexible scheduling options.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
