Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling support.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to adjustment, financial concerns, and end-of-life planning.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
