SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. The role involves providing clinical social work services and psychosocial assessments to patients, addressing a variety of challenges related to institutional living, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services to patients and their families.
• Address common issues such as financial concerns, family relationships, and adjustment to medical diagnoses and treatments.
• Develop discharge plans and offer counseling on end-of-life concerns.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
