SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our compassionate team at LifeBridge Health. The ideal candidate will provide vital clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Deliver expert clinical social work services to patients/residents and their families.
• Address common psychosocial issues such as adjustment, financial worries, and family relationships.
• Support patients in coping with medical procedures, aging-related changes, and end-of-life decisions.
• Assist with discharge planning and counseling on topics related to death and dying.

Requirements

• Bachelor's degree and relevant professional experience.
• LBSW/LMSW/LGPC certification.
• Demonstrated expertise in clinical social work theory and practice.
