Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges and needs. This role involves assisting individuals with adjustment difficulties, financial concerns, family dynamics, medical treatment comprehension, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with various emotional and practical challenges.
• Collaborate with healthcare teams to support patients' well-being and treatment plans.
• Participate in discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Proficient in addressing social and psychological issues in healthcare settings.
• Compassionate, empathetic, and able to communicate effectively with diverse populations.
