SOCIAL WORKER PRN

Job Description

We are currently looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges they may face in healthcare settings.

Responsibilities

• Assess and address common psychosocial issues faced by patients and families.
• Provide counseling and support related to discharge planning, bereavement, and other social work needs.
• Collaborate with multidisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
