Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients/residents and families in coping with medical diagnoses, discharge planning, and end-of-life issues.
• Collaborate with healthcare teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree with professional knowledge in social work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
