Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues. If you are compassionate, empathetic, and knowledgeable in social work practices, we would like to hear from you.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and their families.
• Address concerns related to institutional living, family dynamics, aging, and more.
• Provide counseling on topics such as death and dying, discharge planning, and medical procedures.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Knowledge of social work theory and practice.
