SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges and concerns they may face. This role involves supporting individuals as they navigate medical procedures, discharge planning, aging-related changes, and emotional difficulties.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling and support related to difficult medical decisions, aging, and end-of-life care.
• Collaborate with interdisciplinary teams to ensure comprehensive care for individuals.

Requirements

• Bachelor's degree and relevant professional knowledge in social work.
• 1-3 years of experience in a similar role.
• LBSW/LMSW/LGPC certification.
• Compassionate attitude and strong communication skills.
