SOCIAL WORKER PRN

Job Description

We are looking for a passionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a key member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families.

Responsibilities

• Offer counseling and support for patients and families facing various social and emotional challenges.
• Assist with discharge planning and end-of-life counseling.
• Address financial concerns and family relationships to enhance patient well-being.
• Collaborate with healthcare professionals to create comprehensive care plans.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• LBSW, LMSW, or LGPC certification required.
• Strong understanding of social work theory and practice.
