Job Description
We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more. You will play a crucial role in supporting individuals and families through difficult circumstances.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist with discharge planning and end-of-life care.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Requirements
- Basic professional knowledge with a Bachelor's degree in social work or related field.
- 1-3 years of relevant experience.
- Licensed as a LBSW, LMSW, or LGPC.
