Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. The ideal candidate will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may face. This role involves supporting individuals and families with adjustment issues, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for issues related to institutional living.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to address patient and family needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
