Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common problems including adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Address issues such as adjustment to institutional living, financial concerns, and family relationships.
• Assist with discharge planning and support patients through end-of-life decisions.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
