Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a range of challenges and concerns. This includes assisting with adjustment to institutional living, financial issues, family dynamics, and counseling on various aspects of well-being.

Responsibilities

• Provide psychosocial assessments and clinical social work services to patients and families.
• Offer counseling on issues such as adjustment, financial concerns, family relationships, aging, and end-of-life planning.
• Collaborate with healthcare teams to ensure comprehensive care for patients.
• Assist in discharge planning and facilitate transitions for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience in social work.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Strong understanding of social work theory and practice.
