Job Description

We are seeking a compassionate Social Worker to join our exceptional team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare professionals to ensure holistic patient care.
• Maintain accurate and confidential patient records.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a healthcare setting.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
