SOCIAL WORKER PRN

Job Description

We are currently looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more. The position requires a Bachelor's degree, 1-3 years of experience, and licensure as an LBSW, LMSW, or LGPC.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address challenges related to institutional living, financial concerns, family relationships, and more.
• Assist with discharge planning and provide counseling on death and dying.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a similar role.
• Licensure as an LBSW, LMSW, or LGPC.
