Job Description
We are looking for a compassionate and skilled PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position at Levindale Hospital involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment difficulties, financial concerns, and counseling on end-of-life matters.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients/residents and families.
- Assist in discharge planning and support with financial and family-related challenges.
- Offer support in coping with medical conditions and treatment procedures.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree and a LBSW/LMSW/LGPC certification.
- 1-3 years of relevant experience in social work.
- Strong communication and interpersonal skills.
- Empathy, compassion, and ability to work effectively in challenging situations.
